In the Quality Assurance and Performance Improvement environment, the Home Health Quality Nurse Coordinator partners closely with Quality Control and Infection Prevention experts to hardwire best practices from an award-winning, top-ranked medium-sized hospital featuring advanced technology like Da Vinci robotic surgery, a Level II NICU, and a Level 3 trauma center; the Nurse Coordinator Quality Assurance will help extend this comprehensive care reach across Southern Georgia, aligning with a new surgery center, specialized women’s services, and eight dedicated facilities, including urgent care and rehabilitation, to ensure seamless standards, reliable handoffs, and consistent quality across settings.

Reporting to a Director of Quality with a collaborative dotted line to the Home Health Administrator and Infection Prevention leadership, the Nurse Coordinator Performance Improvement receives robust analyst, educator, and informatics support while working alongside friendly nurses and a diverse interdisciplinary team of therapists, pharmacists, case managers, and physicians; as a Nurse Coordinator Quality and Infection Control, responsibilities include leading PDSA and Lean Six Sigma projects, ensuring OASIS accuracy and HHVBP performance, driving survey readiness and compliance with CMS Conditions of Participation and accrediting bodies, advancing antimicrobial stewardship and home-based infection prevention practices, optimizing clinical decision support and order sets, mentoring staff through education and competency validation, and translating data into action through transparent reporting and frontline-driven improvements.
